MAKEUP

CONTRACT

The contract will be signed by the person [hereby referred to as client] booking the services for a particular session, and will specify date, time, location, and number of people receiving makeup services. All deposits must be paid at the time of contract signing to guarantee each reservation. All prices are subject to change at anytime until contract is signed and a deposit is received. (Once deposit is received your prices are guaranteed to remain the same and are bound by the contract signed by both the client and Melanie Lynne.) All prices include travel within the tri-cities (Kitchener, Waterloo, Cambridge) cities outside the tri-cities are subject to a small travel fee

BOOKING

To secure a date, a signed contract is required along with a 25% non-refundable deposit for all services booked. Please be advised dates and scheduled makeup time will only be reserved once a signed contract and deposit are received. Once deposit is received services cannot be deducted. ____initial

  • full payment of a wedding trial is due on the day of trial

Note: any person(s) who does not have a booked appointment looking to last minute have their makeup done will only be considered after all booked clients are completed and time permits me to do so.

*Early Call Times: A $50 fee will be charged for booked appointment times prior to 7:00am ____initial

LIABILITY

I, Melanie Lynne use only professional grade makeup and tools and ensures that all makeup and tools are always sanitized before use on each and every client. For this reason I, Melanie Lynne am not responsible if the client develops any health issues relating to the makeup application during or afterwards (example: underlying/unknown allergies)

PAYMENT

The final balance is due day of event. All payments are in Canadian dollars. The person(s) responsible for the entire balance of payment is the person(s) who has signed the contract. Acceptable forms of payment are: cash, or Interact E-Transfer. ______initial

CANCELLATION

Client: Cancellations are preferred 14+ days prior to booked date. In the event the client cancels the booking, the client will lose the deposit made to the artist. If the client cancels booking within the 14 days of booked date the client is also subject to a $100 cancellation fee.

Covid-19: Appointments needing to be moved due to covid are eligible for a 1-time date change where the deposit will be moved to new future date.

Makeup Artist: If the makeup artist cancels at any time or is unable to perform her duties for any reason, 100% of the deposit paid will be refunded within two (2) weeks. Client agrees that the refund of 100% of the deposit is the only liability to Melanie Lynne Artistry. Additionally, if the client fails to show up to appointment without contacting Melanie Lynne Artistry via phone email or text the client will pay the full amount of the service scheduled.

SERVICE LOCATION AND REQUIREMENTS

Location of service for the day of the event will be at the digression of the client. Certain requirements the makeup artist will need includes a table/work area, and ample lighting (ideally near a window).

PARKING FEES

Client is responsible for parking fee/valet

TRAVEL FEES

Travel within the tri-cities is included. Locations outside the tri-cities are subject to a travel fee based on kilometres and time travelled.
